Group and Community Volunteer Activity Application

(For Group Leader/Organizer ONLY)


This application is for One-Time Group Volunteer Activities at Children's Mercy and should be completed by individuals age 18 and older who are interested in giving back in a meaningful way, and who are the leader/organizer of their group. A separate application will be filled out by any additional members of your team.

Group and community volunteers have the opportunity to make a significant impact for our patients and families. Activities may include assembling Happy Kits, sharing a special talent or program, supporting the Donation Center, or providing a group meal.

Please take a few minutes to complete the application below so our team can learn more about your group and your interest in serving.


To begin: Please create a User ID and password. Be sure to save your login information, you will need it to complete additional steps in the onboarding process.


*Each member of your group will need to complete a separate application to participate.

After you submit this form, you will receive:

-A link to the group member application

-Your group name (Case-sensitive)

-Your team code (Case-sensitive)


Please share these details with your group so members can complete their application and join your volunteer activity.
